Service description

Merton Road offers accommodation-based service in 2 separate houses intended to be short-term for adult females ONLY aged 18-65 years. The service is for people who live independently in the community but require minimal support. The service is tailored to meet the needs of adults with mental illness, with or without substance misuse, or learning disabilities.

The service;

Actively support people to engage with appropriate local services.

Provide a safe and supportive environment for people we support.

Support people  to live independently in the community.

Provide staff support that is commensurate with the needs of people we support.

Work in partnership with people we support, adult social services, NHS healthcare services, primary care services, alcohol treatment services, community services, and local authorities.

Support available at this location;

Help in establishing personal safety and security.

Developing personal and social living skills.

Developing coping and behaviour strategies.

Help in finding more independent accommodation.

Help in maintaining home security.

Monitoring of health and well-being.

Supervision on medication.

Helping in maintaining tenancy.

Help in gaining to other services.

Help in establishing social contacts and activities.

Access to local communities.

Advice, advocacy and liaison.

Helping in managing finances and benefits claims.

Liaison with mental health services and other relevant services.

Risk assessments.

Support with preparing menus and meals.

Support with laundry.

Support with daily chores.

Advice and support with benefits.

Support with accessing the community.

Support with engaging with providers of appropriate services.

Support with accessing educational, occupational, social and leisure activities.

Monitoring and support with self-medication.

Description of the houses

27 Merton Road is located in the Upton area of Slough, East Berkshire. It is easily accessible with secure off-street parking. There are 5 double bedrooms and 1 has an ensuite bathroom. There is a good size kitchen dinner equipped with a cooker, microwave oven, shared fridge/freezer, dish washer, washing machine, dryer, crockery, and lockable individual food storage cupboards. There is one living room with a television (Freeview). Service users can have televisions in their own bedrooms and they are free to install cable / satellite television at their own expense. Outside there is a lawned garden at the rear.

27a Merton Road  has 3 double bedrooms, 1 bathroom, and 1 toilet. Local area

The house is about 20-minute walk from Slough railway station, and few minutes from Sainsburys supermarket.  Slough is a town located 24 miles west of London. Merton Road is near Slough Job Centre and close to Lascelles, and Upton Court Parks.  Located to the north east is a large area of woodland and lakes that form part of the 43-square-mile Colne Valley regional park. There are nearby sports centres and learning centres, for easy access to recreational, sports and fitness activities as well as a range of council-run events and courses.

Transport

Merton Road is located near junction 6 of the M4. There are local bus routes linked to West London for fast travel directly into London. There are community bus services travelling to local supermarkets, hospital, and train station.
